MY HEALTH | Day Kimball Medical Group (DKMG) offers you the ability to access your health information and communicate with your physician's office - conveniently and securely from your computer.

During your next office visit with either your DKMG primary care practitioner or specialist, you will be asked if you'd like to register for this service. You may also call your physician's office and ask to get started right away.

The service representative at your physician's office will ask you for your email address to begin the registration process. You will be given the option to receive a temporary password via your smartphone or you may opt to receive an email to create a password and complete the registration process for the Patient Portal.
